Ms Evelyn Culnane

Evelyn leads the Transition Support Service at the Royal Children’s Hospital (RCH) Melbourne, which in 2020 received the inaugural Exemplar Practice Award from the Australian Commission on Safety and Quality Health Care.  The Service is an integral hospital-wide service which cares for over 2000 patients annually across all paediatric and adult health service disciplines, general practice and the community. Evelyn’s leadership in the field of transition care has resulted in positive outcomes for patients with complex needs and their families including the implementation of a new model of care for adolescents with an Intellectual Disability and/or Autism Spectrum Disorder with mental health or behavioural concerns.  She is also pleased to chair Children’s Healthcare Australasia’s transition special interest group.
